Who receives it

Your message is stored in the VoteRuiz site owner's private dashboard. Reviewer sign-in is restricted to the owner. Submissions are not posted publicly or provided to other organizations. Cloudflare provides the hosting, database, anti-spam checks and sign-in infrastructure.

What the form asks for

Your message and request type are required. Topics and your name are optional. If you check “I'd like to be contacted,” we ask for the email address or phone number needed for your preferred reply, plus optional availability. Conversation and help requests need a contact method. Do not include patient records, confidential grievance details, or private personnel records.

How contact details are used

Only to respond to your message or arrange the conversation you request. Sending a message does not subscribe you to campaign updates, bulk email, automated texts, or advertising audiences. The form does not book an appointment or send an automatic reply.

Storage and deletion

Message and contact details, and private reviewer notes, are encrypted in the application database. Records are scheduled for removal after 90 days. After a successful submission you receive a private deletion link. Save it and do not share it. It lets you remove that submission's content and contact details from the active application database. Provider backups and infrastructure logs may have separate retention periods.

Site counts and security

The site records aggregate daily event totals, such as page views and successful submissions, using broad source categories. These totals are not joined to individual message records and do not establish unique visitors or support. The footer lets you pause browser event counts; Global Privacy Control and Do Not Track signals are respected. Cloudflare may process IP addresses and connection information for security, service delivery and anti-spam checks. We do not install advertising pixels or create individual browsing profiles. Leaving your name off does not guarantee technical anonymity.

Questions about your information

Use your private receipt link to delete your submission. For other privacy questions, send a question through Member Voice, check the contact box and provide a reply method. A receipt reference can help locate your earlier message; never post your private deletion token publicly.
